- Participate in transitional care as the medical office responsible for patient care coordination. This keeps you making progress toward coming home instead of digging through hospital bureaucracy.
- Make referrals to medical specialists for chronic conditions as needed to ensure you receive the quality care you deserve, and we continue with follow-up care.
- Provide coordinated care with your assisted living facility. Each facility tends to have a different suite of services, so we will coordinate our services with theirs.
